Covid-19 Agreement & Waiver

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
USA Sport Group (USG) has created new protocols and put in place preventative measures to reduce the spread of COVID-19; however, USG cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending any program may increase your child(ren)s risk of contracting COVID-19.

By signing this agreement, I ACKNOWLEDGE THE CONTAGIOUS NATURE OF COVID-19 AND VOLUNTARILY ASSUME THE RISK THAT MY CHILD(REN) AND I MAY BE EXPOSED TO, OR INFECTED BY COVID-19 BY ATTENDING THE USG PROGRAM, AND THAT SUCH EXPOSURE OR INFECTION MAY RESULT IN PERSONAL INJURY, ILLNESS, PERMANENT DISABILITY, AND DEATH. I UNDERSTAND THAT THE RISK OF BECOMING EXPOSED TO OR INFECTED BY COVID-19 AT THE USG PROGRAM MAY RESULT FROM THE ACTIONS, OMISSIONS, OR NEGLIGENCE OF MYSELF AND OTHERS, INCLUDING, BUT NOT LIMITED TO, USG EMPLOYEES, VOLUNTEERS, AND PROGRAM PARTICIPANTS AND THEIR FAMILIES.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself including, but not limited to, personal injury, disability, and death, illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)s attendance at the USG program. On my behalf, and on behalf of my child(ren)s, I HEREBY RELEASE, COVENANT NOT TO SUE, DISCHARGE, AND HOLD HARMLESS USG, ITS EMPLOYEES, AGENTS, AND REPRESENTATIVES, OF AND FROM THE CLAIMS, INCLUDING ALL LIABILITIES, CLAIMS, ACTIONS, DAMAGES, COSTS OR EXPENSES OF ANY KIND ARISING OUT OF OR RELATING THERETO. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of USG, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any USG program.

I acknowledge & agree:

  • to follow all Program safety rules and procedures posted by USA Sport Group prior to and/or during the Program, including but not limited to those posted on the Covid-19 Information Page
  • to abide by any decision made by USA Sport Group, USA Sport Group Coaching Staff and/or Officials regarding the safe participation in the program, in particular in relation to Covid-19. 
  • USA Sport Group coaching staff and/or Officials, Program staff and/or organizers may dismiss you from the Program Site and/or remove the participant from the program without refund or reimbursement of any kind should your behavior and/or the participants be deemed (in the opinion of USA Sport Group) dangerous (including but not limited to putting the safety and wellbeing of participants and program staff at risk), discriminatory, or having a negative impact on the Program, its participants or Program staff.  
  • you and/or the participant is responsible for bringing all PEP, including but not limited to masks and hand sanitizer. 
  • that any facilities provided at the Program location are used by you and/or the participant at your own risk including but not limited to water fountains, bathrooms, and playgrounds. 
  • to complete the Daily Wellness Form every day prior to attending each session, in order to confirm the participant is healthy and has not been exposed to Covid-19
  • to contact USA Sport Group immediately if the participant tests positive for Covid-19 within 14 days after attending of USA Sport Group Program

You understand and agree that USA Sport Group may without prior notice, terminate, suspend, block or modify your access to the Site or Services, cancel your Program registration, and refuse to honor all or pending purchases if USA Sport Group believes you or someone acting on your behalf has violated or is inconsistent with the Terms & Conditions outlined in this Covid-19 Agreement & Liability Waiver, or the law, or have violated the rights of USA Sport Group, or one of our clients, employees, customers, or participants on a Program. USA Sport Group is not required to provide a refund or exchange of fees you have paid if it exercises any of its rights or remedies because you have violated these Terms & Conditions or any of USA Sport Group's rights. In this event, all outstanding fees will be your responsibility and you agree to pay any outstanding balances on demand.

Daily Wellness & Health Form

The Daily Wellness Form must be completed daily prior to the participant taking part in any USA Sport Group Program or session. If after completing the questionnaire/ form the participant is deemed a risk, the participant will NOT be PERMITTED to attend the Program for that day. 

The form must be completed every day that the participant attends the program, for example, if you register for an 8-week program taking place every Tuesday at 10 AM, you must complete the form every Tuesday before 10 AM, that the program is scheduled to run (total 7 submissions). Early submission (other than on a day that your session is scheduled) will be void.

Useful Tips

The programs I selected are not appearing in my cart
  1. Log out of your USA Sport Group Account
  2. Exit out of the webpage
  3. Reload USAsportgroup.com
  4. Sign In with your username and password
  5. Search & select the programs you wish to add
  6. Checkout
I know my password but cannot log into my account

Try resetting your password, to do so, please click the "forgot password" button within the Account Login page (please note sometimes these emails may go into your spam or junk folders).

If directed from a Recreation Department or third-party site you will need to create a seperate USG account(if you have not done so already).

How do I reset my password?
  1. Click the forgot password" button within the Account Login page
  2. Enter your email address linked to the account
  3. Click submit
  4. Locate the USA Sport Group reset password email you received
  5. Click the link in the email (the link is only valid one time, please note multiple attempts to reset the password using the same link will be blocked)
  6. Enter your username (not your email address)
  7. Click submit
  8. Enter a new password, and confirm (please choose a password without any special characters - just letters and numbers)
  9. Click submit
  10. Log in with your Username and New Password
  11. Find a program and register!
  12. Still not working - contact us using one of the methods listed above.
I have forgotten my username, how do I find this?

Please contact us using one of the methods above. To protect your privacy, during this process, you may be asked some quick security questions to verify your identity before we can provide you with your username. If required, during this time we can also reset your password.

I entered all my credit card info but it won't go through
  1. Check your address - make sure it matches the billing address for your credit card
  2. Re-enter your credit card details
  3. Submit your payment for a second time
  4. Still not working? Contact our office via one of the methods listed above so we may assist in resolving the issue
My coupon/ credit voucher code is not working
  1. Check the date the code expires

  3. If the program you are registering for is not listed on the exclusions page however, you are still having difficulties, contact us via one of the methods above and we can check for you
Why do I have to agree to more than one set of Terms & Conditions?

Every program is governed by a particular set of terms and conditions, for this reason if you have selected to purchase multiple products you may find you have to read and agree to multiple program terms and conditions. Everyone purchasing a program regardless of the organization operating the program is required to agree to the USA Sport Group Liability Waiver.

  3. At the top of the page enter the age of the participant and your zip code
  4. Use the filters to narrow your search and find the selected program
  5. Click the blue "More Info" button
  6. The program details page will give you information on how to register and pay for the program.
